About Our Agency

Risen Consultants was founded by Humberto Mena, a Behavior Analyst and Mental Health Counselor with over a decade of experience within his fields. We are a BHCOE accredited ABA service provider.

As ABA professionals, we provide individualized behavior services that suit the needs of our clients and their families through our commitment to the science of Applied Behavior Analysis.

How we were founded

  • Humberto Mena founded our agency in the year 2012.
  • Humberto Mena has a Master’s degree in Psychology with a major in Mental Health Counseling from Carlos Albizu University, graduating with utmost distinction as the valedictorian of his class. Prior to that, he obtained a Bachelor’s degree in Psychology from Florida International University.
  • Humberto has been a Licensed Mental Health Counselor (MH 14969) since 2017 & a Board Certified Behavior Analyst (BCBA 1-16-22872) since 2016. He has over a decade of experience working with both children and adults with several mental and emotional disorders including autism spectrum disorder, ADHD, personality disorders, substance abuse, and mental health disturbances.

Our parent education programs are designed to help parents understand and partake in their child’s intervention plan, and to train and support all family members involved in the daily life of the child. Behavior Analisys:

Risen Consultants provides a variety of Behavior Analysis services through highly-qualified behavior analysts and behavior assistants to meet the unique needs of individuals and their Families through training and consistent consultation. Behavior analysis services are provided to assist individuals to learn new behaviors that are directly related to existing challenging behaviors or functionally equivalent replacement behaviors for identified challenging behaviors. Services may also be provided to increase existing behavior, to reduce existing behavior, and to help the individual emit behavior under precise environmental conditions. The term “behavior analysis services” includes the terms “behavior programming” and “behavioral programs.” Behavior analysis includes the design, implementation and evaluation of systematic environmental modifications for the purposes of producing socially significant improvements in behavior. It includes identification of functional relationships between behavior and environment. It uses direct observation and measurement of behavior and environment. Contextual factors, establishing operations, antecedent stimuli, positive reinforcement and other consequences are used based on identified functional relationships between behavior and environment, in order to produce practical behavior change.

Assessments & Interventions:

Risen Consultants provides comprehensive behavioral intervention for individuals with significant behavioral challenges and/or skill deficits that are impeding on their quality of life. Through proper assessment we can identify the individual’s goals and areas of concern by conducting interviews with the Client and caregivers. After further observation of interviews and activities in specific settings, the information gathered is used to properly design the appropriate and effective Behavior Plan for the client.

Caregiver Educational Classes:

Risen Consultants parent education programs are designed to help parents understand and partake in their child’s intervention plan. It is designed to help parents not only understand but teach the basic principles of the behavior services provided by our team. Just as importantly, it is designed to train and help the grandparents, siblings and all other family members that are involved in the daily life of the child. Following through is a key aspect of your child’s program. everyone involved needs to be dedicated in order to achieve maximum results.

Social Skills Group:

Risen Consultants offers social skills classes that utilize the principles of Behavior Analysis Services under the supervision of a Board Certified Behavior Analyst (BCBA). A specific treatment program is designed by looking at all of the important social skill areas including social referencing, play skills, imitation, joint attention, behavior skills, social awareness and eye contact, as well as conversation and language skills and school readiness skills. The Social skills groups run from 1 hour to 1 ½ hours in duration, depending on the needs of the clients and size of the group. Our goal is to match a child’s needs and skill level to similar-matched peers.

Individual, Groups & Family Therapy:

The primary purpose of therapy is to help treat personal challenges and learn how to best live with or overcome illnesses and disorders that may affect a person’s daily life.

Behavior Analysis Certification Board (BACB) Supervision:

Risen Consultants offers monthly supervision hours that are in compliance with BACB standards

Internship University Supervision:

Risen Consultants provides internship programs for students seeking clinical experience.

Admission Process

Getting Started With Risen Consultants

Who is eligible for our services? Risen Consultants serves children and adults with behavioral challenges. Typically, we support individuals who require assistance in

  • Developing or enhancing language skills to more effectively communicate their needs and functional skills (e.g., daily living, self-regulation) that will lead to greater independence
  • Diminishing problem behaviors such as noncompliance, aggression, self-injury, elopement, and property destruction that limit their ability to develop skills and participate fully in integrated settings.

What is needed to start a services
1.Initial Assessment (Admission):
a. Physician’s written order
b. Medical necessity letter
c. One of the following:

  • Diagnostic evaluation conducted by one of the following:
    Comprehensive Diagnostic Evaluation:A Comprehensive Diagnostic Evaluation (CDE) is a thorough review and assessment of the child’s development and behavior using national, evidence-based practice standards, which may include:

     

    • Parent or guardian interview Teacher assessment
    • Diagnostic testing using tools such as:
    • Autism Diagnostic Observation Schedule (ADOS-2)
    • The Childhood Autism Rating Scale – 2nd edition (CARS2)
    • Modified Checklist for Autism in Toddlers, Revised (M-CHAT-R)
    • Communication and Symbolic Behavior Scales (CSBS)
    • Autism Diagnostic Interview, Revised (ADI-R)
    • Social Communication Questionnaire o Battelle Developmental Inventory- 2nd edition
    • Hearing and vision testing
    • Genetic testing
    • Neurological and/or other medical testing
    • The CDE must be led by licensed practitioners qualified to assess child developmental disorders such as:
    • Developmental Pediatricians
    • Child Neurologists
    • Child Psychologists
    • Child Psychiatrists

Client/ Legal Guardian/ Parent Rights

  • You have the right to receive a notice explaining how a provider or health plan uses and discloses their health information.
  • You have the right to request to review or receive your medical files.
  • You have the right to cancel a release of information by providing us a written notice. If you desire to have your information sent to a location different than our address on file, you must provide this information in writing.
  • You have the right to restrict which information might be disclosed to others. However, if we do not agree with these restrictions, we are not bound to abide by them.
  • You have the right to request that information about you be communicated by other means or to another location. This request must be made to us in writing. You have the right to disagree with the medical records in our files. You may request that this information be changed. If denied, you have the right to make a statement of disagreement, which will be placed in your file.
  • You have the right to know what information in your record has been provided to whom. Request this in writing. If you desire a written copy of this notice you may obtain it by requesting it from Risen Consultants, LLC.
  • You have the right to request an amendment to medical records.
  • You have the right to request special privacy protection for Private Health Information (PHI).
  • You have the right to an accounting of disclosures and to how much information will an accounting of disclosures include.

Public Safety:

  • Health records may be released for the public interest and safety for public health activities, judicial and administrative proceedings, law enforcement purposes, serious threats to public safety, essential government functions, military, and when complying with worker’s compensation laws.

Abuse:

  • If a client states or suggests that he or she 1) is abusing a child or vulnerable adult, or 2) has recently abused a child or disabled adult, or a disabled child or adult 3) is in danger of abuse, the health care professional is required to report this information to the appropriate social service and/or legal authorities. If a client is the victim of abuse, neglect, violence, or a crime victim, and their safety appears to be at risk, we may share this information with law enforcement officials to help prevent future occurrences and capture the perpetrator.

Prenatal Exposure to Controlled Substances:

  • Health care professionals are required to report admitted prenatal exposure to controlled substances that are potentially harmful.

In the Event of a Clients’ Death:

  • In the event of clients’ death, the spouse or parents of a deceased client have a right to access their child’s or spouses’ records.

Professional Misconduct:

  • Professional misconduct by a healthcare professional must be reported by other health care professionals. In cases in which a professional or legal disciplinary meeting is being held regarding the health care professionals’ actions, related records may be released in order to substantiate disciplinary concerns. In judicial or administrative proceedings, health care professionals are required to release the records of clients when a court order has been placed.

Minors/Guardianship:

  • Parents or legal guardians of non-emancipated minor clients have the right to access the clients’ records and may do so at any time.

Complaints:

  • If you have any complaints or questions regarding these procedures, please Risen Consultants LLC. at the address or telephone number listed in the back of this brochure. We will respond to your inquiry in a timely manner. You may also submit a complaint to the U.S Department of Health and Human Services and /or the Therapists State Licensing Agency. If you file a complaint, we will not retaliate in any way

Client Responsibilities:

  • To be actively involved in the development and implementation of Behavior Analysis/Assistant services by being at home at scheduled time, working with the Analyst for the entire session unless directed otherwise, completing assignments as required, making changes in the environment as written.
  • To provide accurate information on all forms and requests for information.
  • To refrain from violent or threatening behavior or language.
  • To refrain from the use of mood altering substances during services.
  • To accept and comply with referrals to another service provider, including Physician consultation
  • To notify of any change including an illness or other emergency.
  • To meet your financial obligations to the BA services as outlined in the Payment Contract for Services.

Client Bill of Rights:

  • Dignity and Equal Treatment: Each client has a right to impartial access to treatment, regardless of race, religion, sex, sexual preference, marital status, veteran status, ethnicity, age or disability. The personal dignity of each client is recognized and respected in all care or treatment provided. The client’s civil rights are protected by federal and state of laws.To discuss your treatment with your doctor or attorney.
  • Privacy: Each client has the right to expect that all treatment records or information will be kept confidential in compliance with Risen Consultants policy except as authorized and as required by law. In case of any disclosure of information not authorized, client will receive an accounting of disclosures of your protected health information. The client will maintain access to all his/her health care records upon request.
  • Religious freedom and practice: Client has the right to choose his/her own religion. Client can choose not to participate in any religion. No one can make client attend church or be a part of any church activity unless it is something the client wishes to partake in.
  • Right to Participate: Each client and/or caregiver is encouraged and has the right to participate in the development of the plan of treatment, evaluate the plan of treatment, request changes and to voice grievances without fear of negative impact on the service provided. An amendment can be requested; however, this request could be denied. If denied, your request will be kept in the records.
  • Right to receive appropriate information: Each client has the right to be informed about the cost of services, what behaviors or violations could lead to termination of services at our agency, receive an investigation about  any complaints.
  • Right to Individualized Treatment: Each client has the right to receive individualized treatment which includes:
    a.Superior Behavior Analyst services regardless of the source of financial support.
    b.Services provided in the least restrictive environment possible.
    c.An individualized treatment plan which is reviewed as needed and have his/her expectations of services assessed regularly.
    d.From someone with training or experiences from a specific cultural,spiritual, or gender orientation.
  • Right to Timely Communication: Each caregiver has the right to timely responses by his/her preferred mode of communication.
  • Right to Highly Trained Staff: Each client has the right to work with highly trained staff members dedicated to his/her child’s program and development who will work within the scope of his/her expertise and under any supervision as laid out by the Guidelines of the Behavior Analyst Certification Board. (BACB).
  • Right to Caregiver Training: Each caregiver has the right to receive training on his/her child’s Behavior Analysis program and Behavior services in terms and language they understand.
  • Right to Request Staffing Changes: Each caregiver or client has the right to make requests regarding staffing changes or appointments and have them addressed in a timely manner.
  • Right to Continuity of Care: Each client has the right to maintain continuity of care. In the event that a change in service provider needs to be made (e.g. relocation, change in funding source, or other) we will support in the transfer of documentation and services. In case of client or caregiver denial of services, agency will inform of any potential consequences.
  • Free from harm, unnecessary restraint, abuse, neglect: Each client has the right to be treated fairly, having someone explain what was done inappropriately and the consequences of his/her actions. Client has the right to report any occurrence to the Abuse Hotline. 1-800-962-2873, if anyone does not treat them humanely.

1.Lead Analyst: BCBA, FL-CBA, LMHC, LSW
  • The Lead Analyst is involved in the implementation and evaluation of interventions with individuals to produce socially significant outcomes. This position is responsible for performing professional work that is substantially varied in nature under general direction from the Clinical Director. The Lead Analyst applies professional training and experience to prepare and implement appropriate interventions for clients, revises assessments, supervises BCaBAs and carries out other appropriate assignments.
2. Assistant Behavior Analyst: BCaBA
  • The Assistant Behavior Analyst (BCaBA) is involved in the implementation and evaluation of interventions with individuals to produce socially significant outcomes. This position is responsible for performing professional work that is substantially varied in nature under general direction from the Lead Analyst or Clinical Director. The BCaBA applies professional training and experience to implement appropriate interventions for clients, and to carry out other appropriate assignments.
3.Registered Behavior Technicians: RBT
  • The Registered Behavioral Technician (“RBT”) is involved in the implementation of interventions with individuals to produce socially significant outcomes. This position is responsible for performing professional work that is substantially varied in nature under general direction from BCaBA and/or Lead Analyst. The RBT applies professional training and experience to implement appropriate interventions for clients, and to carry out other appropriate assignments.Essential components of this position are listed below but are subject to change at the discretion of the company.  This job description describes the essential job functions, but does not provide an exhaustive list of duties that may be assigned.
